What is a Geriatric Care Manager
A GCM is a professional who specializes in assisting seniors and their families with long- or short-term care arrangements. At Generation Solutions, our Geriatric Care Managers have training in gerontology, social work, nursing or counseling and are members of the National Association of Professional Geriatric Care Managers.  GCM members can make contributions in providing comfort and quality of life for the elderly, while relieving stress for the family caregivers.  Call to schedule an in-home appointment to learn complete details.

How Care Managers Can Help

Communication:

  • Act as a liaison to families, in or out of town, by providing constant communication and alerting families to any concerns
  • Provide strategies for coping with Alzheimer's disease and other types of dementia
Professional Assessments:
  • Conduct care-planning assessments to identify problems and develop a care plan for assistance and needed services
  • Review financial, medical or legal issues, offering referrals to specialists to avoid any problems
Service Coordinator:
  • Assist with decisions about living arrangements and all types of placement
  • Arrange and monitor in-home help and other services needed to stay safely at home
  • Monitor home maintenance and repair services to ensure seniors receive the services
  • Assist in a safe transition to home from a hospital or nursing home
Financial Support:
  • Assist with bill paying, personal bookkeeping and provide notary services
  • Control costs by reducing inappropriate institutional care and overused services
